medium is 25 updates per second
low is ~16 updates per second
This doesn't explain the behavior you explained though. Getting updates only every 400 ms with a significantly better ping can only be a sign of a really crappy connection. Bear in mind that the ping is only about speed of the packets. There might also be a huge packet loss ratio.
Also lags and bad connections are absolutely visible in CS2D because there is no player position/rotation interpolation. This might look ugly but you always get the most up to date and precise player state this way.
